We want to share with you some really exciting news about the changes to the
Local 1014 Health Plan. Due
were unable to announce all of the outstanding enhancements to our health plan
in the "Choices" material including our historic orthodontia and "excess" dental
benefits. However, we persevered, fought and eventually won gains in our
medical premium contributions and expanded health trust benefits! Here are the
enhancements:
• A new $2,000 lifetime orthodontia benefit not only for your children,
but for you and your spouse as well.
• A new "excess" dental benefit of up to $1,000 per person per year to
pay for eligible dental expenses that exceed the Delta Dental annual
maximum.
• New and improved We1lness and Disease Prevention benefits for you and
your family including:
o An increase in the annual Wellness Benefit from $550 to $600
o Reimbursement for all medically necessary immunizations
through the Wellness Benefit, regardless of age. Reimbursement
for all flu shots has been made retroactive to September 1, 2009.
o $250 per year for a smoking cessation program
o A reduction in the eligibility requirement for the Medically
Managed Weight loss program, making it available to more
members
o Reimbursement for instruction for self-care and disease
management for newly diagnosed diabetics up to $250
• An improved acupuncture benefit that will provide reimbursement at 90%
of the Anthem Blue Cross rate in-network after the annual deductible is
satisfied.
• An increase in the out-of-network benefit for both chiropractic and
acupuncture that will provide reimbursement of 70% of the reasonable
and customary fee after the annual deductible is satisfied.
• Improved mental health and substance abuse benefits that no longer limit
the number of outpatient visits or inpatient days for medically necessary
care.
