Important Information for Local 1014 Health and Welfare Plan Members Concerning ID Cards

 
Due to a problem at Anthem Blue Cross you will receive two sets of ID cards. The first set will have the prefix to your ID number as LLT, for example the ID number would read LLTFF109xxxx. This number is incorrect and should not be used for verifying eligibility for doctor’s offices, hospitals, prescription drugs, VSP or MHN. You will be receiving an apology letter from Anthem Blue Cross
 
New ID cards are being produced with the ID number that starts with XDH. For example the ID number would read XDHFF109xxxx. This is the number that was on your card prior to the current mix-up. We are working as quickly as possible to get you the new corrected cards. In the interim, if you experience any problems with eligibility, please call Local 1014’s Member Services at 800-660-1014. We apologize for any inconvenience this has caused. Below is pictured a mock-up of what the correct card will look like.
