LACERA Durable Power of Attorney

For your spouses and family, please take some time to complete and mail to LACERA the RETIREMENT SPECIFIC durable power of attorney.

While some durable power of attorney forms in the past have been accepted, some have not.  You must sign and have notarized specifically that you want your designee (attorney-in-fact) to handle your retirement affairs.  This LACERA drafted form eliminates that ambiguity.

Whats important about Power of Attorney?  If you are an active employee, retirement decisions must be made if you are incapacitated and unable to make them.  Power of Attorney allows your designee to make such decisions when you are incapacitated.  Those decisions made by the designee may mean a huge difference in spousal and childrens benefits.

Please take the time to fill it out and send to LACERA so it is on file.
